Your First Dental Visit
Prior to your first appointment, we’ll send you a convenient confirmation email and appointment reminder that will also give you the option to fill out our patient forms using our online portal to help you save time in our office. Once you come to see us, you’ll be escorted to a treatment room where you’ll get to meet your dentist and discuss your current dental issues. After a brief examination, they will recommend treatment and even perform it if time allows. Afterward, you’ll be given post-procedure instructions, and your doctor will call you within 24 hours to make sure you are recovering well.
New Patient Forms
Please click on the link below to fill out our new patient forms using our online portal. This will help you save about 15-20 minutes when checking in for your appointment. Once they are completed, they’ll be sent directly to our office so when you come to see us, we can get you in to see your dentist right away! We truly respect your time and never want to leave you waiting too long to see your doctor. If you have any issues accessing our forms, be sure to let us know.
Dental Insurance Welcome
In addition, we work with all PPO insurance providers to help maximize your dental benefits. Do keep in mind that most dental insurance plans differ vastly from medical plans and are often very limited in their coverage. When you use your dental insurance with us, we’ll file the claim on your behalf, handle all of the paperwork, and do any necessary follow-ups to ensure that you’re getting the absolute most out of your benefits.
Our care is provided to you, our patient, and not to an insurance company. The insurance company is responsible only to the patient as a benefit from the individual or employer plan that has been purchased, and the patient is responsible to the doctor. We will assist you in every way we can by filing your dental or medical claim and handling questions from your insurance provider to our office on your behalf.
CareCredit Financing Available
If you currently do NOT have insurance or require a treatment that is not covered by your plan, then we can help you get flexible, low-to-no interest financing with CareCredit. With a CareCredit plan, the cost of your treatment will be broken up into much smaller monthly installments. In an effort to reduce the cost to you, the patient, while maintaining the highest level of professional care, we have established a financial policy as follows:
- Payment of any account balance is due in full within 30 days of the statement date.
- We gladly accept Visa, Discover, American Express, MasterCard, Cash, and Check payments.
- In most cases, your entire treatment will be completed in just one visit, therefore, we respectfully request your copayment in full at the time services are rendered.
- As a courtesy to our office, if you need to reschedule an appointment, please notify us within 36-hours of your visit to avoid a cancellation fee of $125.
